Final Submission Notices:

  • Michigan State University requires several forms to participate in youth programs on campus. These forms will be emailed to you after registration and must be completed by a parent or guardian before students may attend.
  • A final confirmation form will be emailed to your primary contact listed, in order to sign up for specific sessions, indicate any dietary restrictions, and provide student rosters for name badges.  Submitting this confirmation form by May 2, 2025 will be required to complete your registration. 
  • Please note that photos will be taken at this event.
